American Traveler is hiring an experienced RN Case Manager with an active Alaska license and at least 2 years of recent case management experience.

Details

• Clinic-based setting providing case management and care coordination
• Monday through Friday, 8am-5pm schedule
• Ambulatory and specialty clinic patient population
• May serve as OB case manager and support specialty clinics
• Patient care involves both acute and chronic needs, with multi-disciplinary collaboration

Requirements

• Active and unrestricted Alaska RN license required at time of consideration
• Minimum 2 years of recent case management experience within the last 4 years
• Graduate of an accredited school of nursing; BSN preferred
• Current BLS certification required; ACLS preferred
• Experience in ambulatory, clinic, or specialty case management settings preferred
• Proof of flu vaccination required upon consideration

Additional Information

• Responsible for managing care plans, coordinating multi-disciplinary care, and facilitating referrals for complex patients
• Provides patient education, conducts outreach, and manages health maintenance through review of EHR reminders and triggers
• Acts as a key advocate for patients and their families to support independence and self-care
• Supports specialty clinics and may assist in setup and clinical procedures
• Collaborates with a team-oriented environment focused on patient outcomes and interdisciplinary improvement efforts
• Participates in ongoing competency validation, including safety and infection control
