American Traveler is seeking an experienced RN Care Manager with trauma setting experience and certification for a weekday, inpatient case management position.

Details

• Work in inpatient units within an acute care hospital
• Weekday schedule: Monday through Friday, 8:00am to 4:40pm
• No weekend or on-call responsibilities
• Caseload management of diverse inpatient care needs
• Attend interdisciplinary rounds and collaborate on care planning
• Utilize Epic EMR for patient documentation and orders

Requirements

• Active NH or compact RN license required
• Certification as a care manager required
• Bachelor of Science in Nursing (BSN) required
• Minimum 3 years of inpatient clinical experience
• At least 1 year of care management experience required
• Recent Level 1 or comparable Level 2 trauma experience within the last 2 years required
• Current Basic Life Support (BLS) certification is mandatory
• Candidates must reside outside a 75-mile radius—no local candidates allowed

Additional Information

• Responsibilities include initial assessments, developing and facilitating care plans, and collaborating with patients, families, and healthcare teams
• Frequent communication with medical staff and post-acute referral sources to coordinate discharge needs such as home health and inpatient rehab
• Place and document orders for home health, DME, and nutrition in Epic
• Attend interdisciplinary rounds as part of the care coordination process
