American Traveler is hiring an experienced RN for a Manager - Case Management position requiring a BSN, at least 2 years of supervisory experience, and 3+ years in case management or relevant nursing practice.

Details

• Hospital-based Case Management department
• Day shift: 8:00 AM – 4:30 PM
• 40 hours per week, 8-hour shifts
• 16-week contract
• Weekend coverage required
• Holiday coverage required (1 major, 1 minor)
• On-call required approximately 3 times per week
• Epic Case Management module used

Requirements

• Active VA or Compact RN license required
• BSN required
• Current BLS certification required
• Minimum 2 years of direct supervisory or management experience required
• Minimum 3 years of case management or relevant nursing experience required, including utilization review, discharge planning, outcomes management, transitional planning, assessment, care planning, and service implementation
• Epic Case Management module experience required

Additional Information

• Leads and supervises case management staff across the continuum of care, including care progression, discharge planning, and transitions of care
• Collaborates with interdisciplinary teams, physicians, patients, and families to develop and implement individualized care plans
• Ensures timely referrals to appropriate next level of care (e.g., acute care, home health, hospice, SNF, LTAC, clinics, TCM) and coordinates medical equipment and supply orders for discharge
• Participates in interviewing and hiring processes, including developing behavioral interview questions and evaluating candidates
• Monitors and evaluates team performance metrics, productivity, and length of stay data to achieve operational goals
• Facilitates family meetings and proactive communication with healthcare team members to address patient and family needs
• Provides staff education on care plan documentation, variance management, and individualized patient care
• Assures equitable staffing coverage across all assigned units daily
• Participates in clinical performance improvement activities
