American Traveler is hiring a travel RN Case Manager for an inpatient case management position in Chicago, IL, requiring 3 years of acute care case management experience and an active Illinois RN license.

Details

• Inpatient case management role at a 205-bed hospital in Chicago, IL with no trauma designation and a Level II NICU
• Serves an underserved adult and geriatric population, including unhoused and low-income patients with complex social and medical needs
• Case assignments may span PCU, Med/Surg, and LDRP units; ICU assignment is uncommon but possible
• Assigned to a specific unit with the possibility of floating to other inpatient units
• Typical caseload of 14–20 patients
• EMR: Epic (experience preferred); Careport used for referrals
• Monday–Friday, 8-hour day shifts, 0800–1630; occasional late stays may be required
• Every 5th weekend rotation required; when working a weekend, a Monday and Friday off are provided in return
• No on-call responsibilities
• 40 hours per week standard schedule

Requirements

• Active Illinois RN license required
• ASN or higher education accepted
• Minimum 3 years of acute care case management experience required, with most recent experience within the last year
• Strong assessment, critical thinking, and interpersonal skills required
• Demonstrated ability to collaborate with multidisciplinary teams and physicians
• Experience navigating various insurance plans preferred
• At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN collected within the last year required for consideration
• Candidate's date of birth and last 4 digits of SSN required for consideration

Additional Information

• Responsibilities include inpatient care management, coordinating with a multidisciplinary team (including hospitalists available 24/7), and identifying and securing appropriate resources for patients
• Supported by department leadership, fellow case managers, a CM assistant, and 24/7 hospitalist coverage
• Orientation includes facility new employee orientation (NEO) plus 3 department-specific shifts, with 1–2 additional shifts available if needed
• Holiday coverage expected for approximately 50% of holidays (up to 2) during the contract
• Eggplant-colored scrubs or business casual attire required; white lab coat required
