American Traveler is hiring an experienced RN for a Hospice/Palliative Care Case Manager position in the Fairfax, VA territory, requiring at least 1 year of recent hospice experience and a VA or compact RN license in hand.

Details

• Home hospice setting serving the Fairfax, VA territory
• Geriatric patient population requiring end-of-life care
• Caseload of approximately 5–7 patients per day (to be confirmed during interview)
• Case management role requiring flexibility across the assigned territory, up to a 60-mile radius from the hub/office
• Uses Netsmart EMR
• Day shift schedule, 5x8-hour shifts (8:00 AM – 5:00 PM)
• No on-call responsibilities
• Local candidates are accepted at the travel rate

Requirements

• Active VA or compact RN license currently in hand
• Current BLS certification required
• Minimum 1 year of recent hospice nursing experience required
• Case management experience required
• Completion of both applicable skills assessments required for consideration
• Two professional supervisor references from within the past 2 years required for consideration

Additional Information

• Provide case management and end-of-life nursing care to geriatric hospice patients in a home/field-based setting
• Timely documentation and communication are essential to this role
• Flexibility with territory coverage is expected
• An iPad is provided for use in the field; personal cell phone is not required
• Mileage is reimbursed at the IRS standard rate
• Scrubs in any color are required; jeans are not permitted
• All time-off requests must be submitted at the time of initial application
