American Traveler is seeking an experienced RN Hospice Case Manager for a community-based role in Sheboygan County, WI, requiring a minimum of 2.5 years of hospice case management experience and Epic Remote Client proficiency.

Details

• Community-based hospice case management role serving patients in home, assisted living facility (ALF), and skilled nursing facility (SNF) settings across Sheboygan and Manitowoc Counties
• Patient population consists of adults receiving end-of-life care
• Caseload of 13–15 patients at a 1:1 ratio
• Full-time, 40-hour work week on an 8-hour day shift, Monday–Friday, 8:00 AM–4:30 PM
• One weekend shift rotation per month required
• One on-call shift per month required
• No holiday coverage required
• May float to assist Manitowoc County on occasion
• EMR: Epic Remote Client (experience required)
• IV pump: Sapphire

Requirements

• Active WI or compact RN license required
• Current BLS certification required
• Minimum 2.5 years of hospice-based case management experience required, with most recent experience within the last year
• Epic Remote Client experience required
• Valid driver's license and auto insurance required
• Proficiency in end-of-life care and planning, wound assessments, care coordination, pain and symptom management, and triage
• Candidate's middle name or initial, date of birth, and last 4 digits of SSN required for consideration
• Minimum one professional reference from a Supervisor, Manager, Director, or Charge RN, dated within the last year, required for consideration

Additional Information

• Responsibilities include hospice case management, patient assessments, care coordination, pain and symptom management, and triage across home and facility-based settings
• Supported by a team of RNs, Case Managers, and an on-call Manager
• Orientation of up to one week, based on comfort level
• First-time travelers are welcome if they have hospice and Epic Remote Client experience
• Former employees of Advocate Health or any of its subsidiaries must be separated for at least 12 months before being eligible
• Local candidates are accepted
• Scrub color is open or business casual attire acceptable
