American Traveler is hiring an experienced RN Case Manager for an acute care hospital position requiring discharge planning experience, Epic proficiency, and availability to float across multiple campuses.

Details

• Level II Trauma Center with 571 total staffed beds across two campuses
• Case Management unit with responsibility across all areas of the hospital, including two campuses located less than 2 miles apart
• Patient population includes acute care, med/surg, long-length-of-stay (LLOS), and ED cases
• Caseload ratio of approximately 1:25–30
• Work involves care coordination, discharge planning, needs assessments, DME ordering, plan of care development, and CMS/regulatory compliance
• 4x10 schedule, day shift (7:30 AM – 6:00 PM)
• Every other weekend required
• Floating required across all areas of the hospital and between campuses
• EMR: Epic (required)
• Parking is facility-paid

Requirements

• Active WA RN license or Compact RN license (pending license accepted)
• BLS certification required
• Minimum 1 year of acute care experience required
• Discharge planning and care coordination experience required
• Experience with Epic EMR required
• Diverse case management experience across med/surg, LLOS, and ED settings strongly preferred
• ICU and ED experience preferred
• At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty/setting required for consideration
• SSN and DOB required for consideration
• Must reside more than 50 miles from the facility to be eligible
