American Traveler is hiring an experienced RN Hospice Case Manager for a community-based hospice role in the greater Milwaukee, WI area, requiring at least 2 years of hospice experience and Epic proficiency.

Details

• Community-based hospice setting servicing North Milwaukee and the counties of Washington, Ozaukee, Waukesha, and Jefferson
• Adult end-of-life patient population with a 1:1 patient ratio
• Floating may be required within the serviced counties
• Sapphire IV pumps used in patient care
• Epic Remote Client EMR — experience preferred
• Monday through Friday schedule, 8-hour day shifts (0800–1630); four 10-hour shifts may also be available
• No weekend, on-call, or holiday requirements

Requirements

• Active WI RN license or valid compact RN license required
• Current BLS certification required
• Valid driver's license and active auto insurance required
• Minimum 2 years of hospice nursing experience required
• Proficiency in end-of-life care and planning, wound assessments, care coordination, pain and symptom management, and triage
• At least 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN
• Candidate's middle name or initial, date of birth, and last 4 digits of SSN required for consideration

Additional Information

• Provides direct hospice case management to adult end-of-life patients in a community setting
• Supported by a team of RNs, a Resource Nurse, and Case Managers
• Orientation of up to 1 week based on individual comfort level
• First-time travelers are welcome if they have hospice experience and Epic Remote Client experience
• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible
• Local candidates are accepted at the same rate — no radius restrictions apply
• Scrubs in any color or business casual attire are acceptable
• A clinical interview with the manager is required prior to any offer
