Description

American Traveler is seeking an experienced RN for a Clinical Transplant (Post-Transplant) position at a major academic medical center, requiring a minimum of 2 years of Med/Surg or ICU experience along with transplant experience.

Details

• Outpatient clinic-based setting within a large academic medical center (500+ bed facility)
• Post-transplant patient population, including heart transplant recipients
• Responsibilities include attending transplant clinics in person on site
• Day shift, 8-hour shifts: 8:00 AM – 4:30 PM, Monday through Friday
• Schedule is 3 days per week with flexible days (not fixed)
• Epic EMR experience preferred

Requirements

• Active MD or Compact RN license currently in hand
• Current BLS certification required
• Minimum 2 years of recent, strong Med/Surg or ICU nursing experience required
• Transplant experience required
• Experience at a facility with 500+ beds required
• Strong patient triage skills, including telephone triage for both simple and complex problems
• Proficiency with computer data entry and legible documentation
• SSN and DOB required for consideration
• 1 professional reference required for consideration

Additional Information

• Coordinate care and clinically manage post-transplant recipients, including prescription refills, medication reconciliation, and chart review
• Triage patient calls and manage clinical concerns by telephone
• Collaborate with interdisciplinary transplant team including pharmacy, social work, and nutrition
• Heart transplant experience is strongly preferred
