American Traveler is hiring a Home Health RN Float requiring MA RN licensure and 1 year of certified home health care and case management experience.

Details

• Home health setting covering the North Region including Chelsea, East Boston, Revere, and Winthrop
• Float position requiring travel to patient homes as assigned across the coverage area
• 13-week assignment with a schedule of 08:00-16:30
• Weekend rotations required once every 6 weeks
• Uses Epic for charting and documentation
• Uses UKG/Kronos for timekeeping

Requirements

• Active MA RN license required, with no conditional or pending licenses accepted
• NURSYS and BON primary source verification required for nursing license
• BLS certification required (AHA only)
• Minimum 1 year of experience in a certified home health agency required
• Minimum 1 year of case management experience required
• Experience in hospice, private care, or standalone case management roles does not meet the certified home health care experience requirement
• Flu vaccine required with no religious or medical exemptions allowed
• Ability to travel to patient homes as assigned

• Assess patient conditions, implement care plans, and provide ongoing education and support to patients and families
• Perform admission processes and case management responsibilities in accordance with organizational policies and standards of practice
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Deliver patient care within organizational philosophy, policy, and community health practice standards
• Local candidates must have a permanent residence within 60 miles of Massachusetts General Hospital in Boston, with a reduced rate for local candidates
• No validated parking available at Boston locations; public transportation or paid public parking (approximately $17-$22 daily) required
