American Traveler is hiring a travel RN Case Manager for an acute inpatient care coordination role in Silver Spring, MD, requiring an active MD or Compact RN license and Epic experience.

Details

• Acute inpatient hospital setting focused on discharge planning and care coordination
• Collaborates with hospital-based physicians, nursing staff, utilization review, and ancillary team members
• Works with patients, families, and community providers to develop and coordinate safe discharge plans
• Coordinates transitions of care with community providers and ambulatory case managers to prevent readmissions
• Day shift: 8:30 AM – 5:00 PM (8-hour shifts)
• 40 hours per week
• Uses Epic EMR
• 14-week contract

Requirements

• Active RN license — Maryland or Compact license required and must be in hand at time of consideration (pending licenses not accepted)
• Active certifications required at time of consideration (no pending certifications accepted)
• Experience in case management, discharge planning, or care coordination in an acute inpatient setting
• Epic EMR experience
• Last 4 digits of SSN and date of birth required for consideration

Additional Information

• Responsible for overseeing the management and coordination of care for the acute inpatient population, including facilitating appropriate level-of-care transitions and discharge planning
• Collaborates with rounding physicians, patients and families, nursing, and utilization review to ensure clinically appropriate and cost-effective care progression

• Contingent workers are subject to a tenure policy limiting assignment time to no more than two years (including assignments with lapses of less than three months) before a mandatory 90-day break is required
