American Traveler is hiring a Home Health RN with MDS/OASIS assessment experience and an active MA RN license for a community-based assessment-focused role.

Details

• Home Health unit within a community healthlink setting
• Day shift, 5x8 hours, typically 8:00-16:00 though hours may vary between 8-4 and 9-5 depending on the day
• Primary focus is completing MDS or OASIS assessments in patients' homes and/or office settings
• RN works alongside a care partner who typically has an established history with the patient, except for new admits
• No hands-on patient care required
• Responsibilities include completing assessments for insurance/programs, developing care plans, and may include medication reconciliation
• Must be willing to float to all CHL locations within a 38-mile radius
• 40 hours per week guaranteed
• Union facility with no special scheduling accommodations available

Requirements

• Active MA RN license required (permanent, in hand)
• BLS certification required
• Minimum 1 year of recent RN experience required
• Experience with MDS or OASIS assessments required
• LTC or similar experience considered if documented assessment experience is included
• Reliable transportation required, as RN may drive independently or carpool with care partner
• Psychiatric experience helpful but not required
• Must be willing to work flexible hours as needed

Additional Information

• Care partner provides support during assessment visits
• Position has a hard end date, though an extension is possible
• Preference given to candidates with no requested time off (RTO) during the assignment
• No RTO allowed during the first week of the assignment; up to 14 days may be submitted for review
• Must be able to work all 7 days as needed due to union scheduling requirements
• Local candidates accepted within a 200-mile radius at the same bill rate
• Returning staff must have a 12-month separation from the health system
• Required to complete a pass/fail EKG test during orientation with a passing score of 80% and only one attempt allowed
• A pass/fail medication exam is also required at orientation
• Additional compliance modules are completed pre-start, during orientation, and annually
